Nurse Case Manager - Assisted Living Facility

**Salary: $28/hr-$32/hr - based on experience**

Qualifications:

• Must have current New York LPN or RN license
• Must have prior experience as a Case Manager
• Must have experience in Assisted Living facilities
• Strong written and verbal communication skills

Responsibilities:

• Communication with resident families with respect to updates on care status/ change of conditions instructions
• Coordinate with families on resident well-being and status
• Follow up on medical evaluation paperwork with resident providers
• Establish strong working relationship with residents and promote self-centered care
• Oversee the activities department to ensure residents social needs are being met
• Participate in care service meetings
• Assist with coordinating medical, social, and personal care for residents
• Assist residents/ families with Medicaid and other financial programs that they are eligible for to meet care needs
• Submit 3559’s forms to DSS upon admission/discharge/rehab and etc
• Understand and coordinate the rep payee process
• Assist with SSI/ SSP matters and coordinate telephone interviews with agencies
• Stay updated with respect to public assistance programs and help develop policies accordingly
• Assist in the marketing efforts with respect to potential residents including updating CRM system
• Keep files organized and updated
